    ADSL Forms, Agreements, and Contracts

    In order to receive ADSL service through Internet Nebraska, some additional forms need to be filled out in hardcopy. If you sign up via the ADSL signup page, you will not need to download and print these documents.

    One agreement with Internet Nebraska needs to be signed and returned to our office, and if you are signing a term contract (eg. to get the free router promotion offer when and where available) you will need to also fill out and return Windstream's ADSL contract.

    Modem Rebate Form

    If you are going to do a one year contract to get the modem for free, you need to get the modem rebate form. Click here to download it. Fill it out and mail it back within the first 45 days to get your $50 rebate.


    In the unfortunate event that ADSL service is not available in your area, you will be notified, and your application will be kept on file. When ADSL service is available in your area, you will be notified again, and asked if you would still like to sign up for ADSL access at that time.

    It can take approximately 2 weeks from the time ADSL service is ordered to the time Windstream is able to install the ADSL service on your phone line. We will let you know as soon as we have more information on the state of your ADSL application. Please direct all ADSL status questions to manager@inebraska.com.
